	Residential Users (Owners and Tenants) Outside the City - $35.00
	Business with Low Water Usage - $50.00
	Car Washes, Restaurant and Hotel, Motel, and Apartment Complexes with Less than 50 Rental Units - $250.00
	Hotels, Motels and Apartment Complexes with 50 or More Units - $500.00
	Any Other Business with Water Usage of 3,000 Gallons a Day or More - $500.00
I hereby make an application for water and/or sewer service at the above location and agree to abide by all rates, ordinances and regulations pertaining to such service. I further agree to pay promptly for all water and sewer services and to continue liable for all charges on said premises until notice has been given to the Director of Finance to terminate this agreement. Notice of Termination must be given before moving out.
