Application

Download the Virginia Voter Registration Application (PDF). You will need the Adobe Acrobat Reader program installed on your computer to view and print the application. Once downloaded, you may print the form, fill out completely and sign the form, and then mail it to us. This form may not be faxed to be accepted.

Change of Name / Address

Please report a change of name or address. To remain a qualified registered voter, the law requires you to notify your local voter registration office of any change in your name or address. This must be done before the registration books close for the next election. You can submit your notice of change of name or address either by fax to 757-220-6157 or by mail:
Office of the General Registrar
P.O. Box 3538
Williamsburg, VA 23185

In either case, the notice must include your signature to be valid. Notice cannot be made by telephone or by email. After your change is processed, you will be mailed a new Voter Information Card. If a new card does not arrive within a few weeks, please call us at 757-220-6157 to confirm that your notice was received.
